If you prefer to send the enrollment form and payment via the mail, you can download an enrollment form in pdf format You will need to send the form and a check to:

Enrollment fee is non refundable after the first class

Please bring proof that your dog is up to date on all vaccinations including Bordatella on the first night of class.
